Survey ID's not unique?

I am trying to pull data out from opinio database to move it to our home db but found out that the survey Id's in table OPS_Survey are not unique. Some surveys have more than one Id. How to pull data out for a specific survey?

what do you mean some surveys have more than one id? SurveyId column in this table should always be unique.

It seems that some surveys are listed in the table and have more than one id. For example, Student survey has three different entries in that table with three different id's although I know that I created it only once. It seems that the createdDate is different for each entry for this survey. Example of OPS_Survey table:
SurveyID SurveyName createdDate createdBy
1120 Student survey 1255014727198 1
1121 Student survey 1255454727198 -2
1122 Student survey 126784727198 -2

Why same survey is having more than one entry in the OPS_SURVEY table?
