Multiple selections for other

I'm creating a nominating ballot and voters have the choice to write-in multiple names. I've seen this posted before and wondering if it is now possible to create a question that would look like.
Please select two choices for your class representative.
A. Bob Smith
B. Valerie Jones
C. Kristine Adams
D. Other _____________
E. Other _____________
Or must I have all write-in options on a single other line?
