Reports Customization

we've had the software for a few years now and our clients would like to customize the reports.
Customizations needed:
1. On the Table of Contents page, state the question and page number instead of identifying the question numerically with page number
2. Include the question on the same page with the frequency table
3. Continuous listing of questions and frequency tables on page instead of one table per page
Are these customizations possible and if so how?
