Solution There is no special place to add custom JavaScript to the survey. However, it can be added to survey header or footer (survey look/feel -> survey texts page), introduction and question-text. Before version 6.9, it is very important that the whole JavaScript block is on one line (there should be no line breaks in the JavaScript!).

The text box you enter the JavaScript into should be in "simple" mode.
